How does Captopril / Generic of Capoten work?

Capoten is an Angiotensin converting enzyme inhibitor (ACE inhibitor). ACE inhibitor inhibits the conversion of Angiotensin I to Angiotensin II. Angiotensin II is an octapeptide in plasma, which is obtained by a precursor plasma alpha globulin and is involved in electrolyte, blood volume and pressure homeostasis. Captopril increases plasma kinin levels and abolishes the presser action of Angiotensin I receptors.

Guidelines before using Captopril / Generic of Capoten

Captopril should be administered one hour before food. The patient should be advised to limit the sodium intake if Capoten alone is used for the treatment of hypertension. The dose of Capoten should be reduced in patients with renal impairment. In patients with renal impairment Captopril administration can cause hyperkalemia. Captopril administration can precipitate hypotension in patients who are on diuretics or who have dietary salt restriction. The patient should be advised to discontinue the diuretic or increase the salt intake one week before Captopril treatment. The patient should be kept in supine position and should administer normal saline to treat transient hypotension. It is not advisable to consume Capoten during pregnancy as it can lead to fetal death.

The recommended starting dosage of Capoten is 25mg twice or thrice daily. The dose of Captopril can be increased to 50mg after one or two weeks if patient does not show sufficient clinical response. The maximum dose of Captopril is 450mg. The dose of Captopril in diabetic nephropathy is 25mg thrice daily.
